Corrections and removals

If something on these pages is wrong, tell us and we will deal with it. Write to corrections@tagproof.io with the facility name, the state, the identifier shown on the page, and what is wrong. Say that it is an assisted living page — the nursing home index has its own route, and the two records are handled separately.

What we can change

  • Annotate.A note on the page saying the operator disputes a finding, in the operator’s words.
  • Fix our arithmetic. If a count or comparison is wrong by our own definitions, it is fixed and the fix is noted.
  • Take a page down. On request from the facility, the page is removed and stays removed.

What we cannot change

The findings themselves. Each page republishes a state agency’s record verbatim — for Florida, the Florida Agency for Health Care Administration; Texas, the Texas Health and Human Services Commission; California, the California Department of Social Services, Community Care Licensing Division; Arizona, the Arizona Department of Health Services, Division of Licensing; Oregon, the Oregon Department of Human Services, Aging and People with Disabilities, Safety, Oversight and Quality Unit; Georgia, the Georgia Department of Community Health, Healthcare Facility Regulation Division; Virginia, the Virginia Department of Social Services, Division of Licensing; Iowa, the Iowa Department of Inspections, Appeals, and Licensing, Health Facilities Division; Utah, the Utah Department of Health and Human Services, Office of Licensing (Division of Licensing and Background Checks); West Virginia, the West Virginia Office of Health Facility Licensure and Certification. A finding that is wrong at the source is corrected with the agency, and when the state’s record changes, these pages change with it at the next monthly pull. We will annotate in the meantime.

How long it takes

Acknowledged within two business days. Resolved, or explained, within ten.
