HFRD prints a scope-and-severity letter beside each citation, on the grid the federal nursing home survey uses: A to C is a deficiency with no actual harm and the potential for minimal harm, D to F no actual harm with the potential for more than minimal harm, G to I actual harm that is not immediate jeopardy, and J to L immediate jeopardy; within each band the letter rises from isolated to pattern to widespread. D is by far the most common.

111-8-62-.21(7) A home must maintain a three day supply of non-perishable food and water for emergency needs. The quantity of food required to be stored must be based on the usual resident census. The food must be kept in sealed containers which are labeled and dated. The food must be rotated in accordance with shelf life to ensure safety and palatability. Water sufficient for drinking and food preparation must also be stored.
