Community Safety Precautions

An assisted living survey tag from HFRD, cited on Georgia licensure inspections and complaint investigations. TagProof groups it under Life safety — one of the building-related survey tags, usually the maintenance side's to fix. Each citation carries a scope and severity set by HFRD.

33% of the facilities cited on 111-8-63-1314 have been cited on it again at a later visit. A finding that recurs is usually a gap in the routine that produces the record — the same check missed, or done and not documented in a form the surveyor accepts — rather than a new fault.

How HFRD classifies it

HFRD prints a scope-and-severity letter beside each citation, on the grid the federal nursing home survey uses: A to C is a deficiency with no actual harm and the potential for minimal harm, D to F no actual harm with the potential for more than minimal harm, G to I actual harm that is not immediate jeopardy, and J to L immediate jeopardy; within each band the letter rises from isolated to pattern to widespread. D is by far the most common.
