Illinois — assisted living inspection records

Every facility the Illinois Department of Public Health (IDPH) licenses as an assisted living or shared housing establishment, with the tags from its inspections and complaint investigations since October 2023. IDPH surveys an assisted living or shared housing establishment for licensure every year, and investigates complaints and the incidents establishments report themselves in between. IDPH's portal lists each establishment's surveys with the statement of deficiencies where one is posted; this record reads the last five years and compares the last three. A survey without a statement is listed as citing nothing once IDPH has closed it.

Not in this record: Fire and life safety inspection, severity, the surveyor's narrative, and enforcement. The fire inspection of an Illinois establishment is the local fire authority's and is not part of IDPH's record; what IDPH itself cites about the building — its physical plant, environmental and disaster preparedness rules — is here. IDPH's statements carry no severity grid, and the plan of correction is the establishment's own text and is not republished. Fines and licence actions are not republished, and the administrator IDPH lists is not named. The portal gives no licence date, so Illinois has no newly licensed list.
