Indiana — assisted living inspection records

Every facility the Indiana Department of Health (IDOH) licenses as a residential care facility, with the tags from its inspections and complaint investigations since October 2023. IDOH surveys a residential care facility for re-licensure about once a year and investigates complaints in between, with revisits to check that findings were corrected. IDOH's consumer report for each residential care facility lists its surveys since December 2008, each linked to its report, posted four to six weeks after the survey once the plan of correction is approved; this record reads the last five years and compares the last three.

Not in this record: Fire safety inspection, severity, the surveyor's narrative, and enforcement. The fire safety inspection of an Indiana residential care facility is the State Fire Marshal's and is not part of IDOH's record; what IDOH itself cites about the building — its sanitation and safety and physical plant standards — is here. IDOH's reports carry no severity grid, and the plan of correction is the facility's own text and is not republished. The administrator and director of nursing IDOH lists are not named. Residential beds licensed inside a comprehensive care facility are part of that nursing home's report and are not included. IDOH's reports give no first-licence date, so Indiana has no newly licensed list.
