Wisconsin — assisted living inspection records

Every facility the Wisconsin Department of Health Services, Division of Quality Assurance (DQA) licenses as a community-based residential facility or a residential care apartment complex, with the tags from its inspections and complaint investigations since October 2023. DQA's Bureau of Assisted Living surveys a community-based residential facility on a risk-based cycle, most every two years, and investigates complaints and the facility's own incident reports in between; a verification visit returns to check that cited deficiencies were corrected. DQA's Provider Search shows each facility's visits of the last three years, each with the statement of deficiencies it issued; these pages use those three years. Visits that age off DQA's page stay in this record once it has read them.

Not in this record: Fire and building code inspection, severity, the surveyor's narrative, and enforcement. The fire and building code inspection of a Wisconsin facility is the local fire department's and the state building authority's, and is not part of DQA's record; what DQA itself cites about the building — its physical environment, building maintenance, building support systems and fire safety rules — is here. DQA assigns no severity on these statements, and the plan of correction is the facility's own text and is not republished. Forfeitures and orders are issued in separate enforcement letters and are not republished, and an owner that is a sole proprietor is not named.
