North Carolina — assisted living inspection records

Every facility the North Carolina Department of Health and Human Services, Division of Health Service Regulation (DHSR) licenses as an adult care home, with the tags from its inspections and complaint investigations since October 2023. DHSR's Adult Care Licensure Section inspects an adult care home every year, with complaint investigations and follow-up visits in between and county Department of Social Services monitoring; DHSR's Construction Section inspects the building and its fire safety every two years. DHSR's page for each adult care home lists every inspection posted since November 2014, with the statement of deficiencies where any were cited; this record reads the last five years and compares the last three. An inspection that cited nothing is listed where DHSR lists it.

Not in this record: Severity, the regulation text, the surveyor's narrative, the star rating, and penalties. DHSR assigns no severity on these statements; the plan of correction is the facility's own text and is not republished, and the star rating and the penalties DHSR lists on the same page are not republished. What is here is unusual among states: DHSR's Construction Section inspects the building and its fire safety itself and posts those statements per home, so this record carries a real life safety section. Family care homes, two to six beds, are also licensed by DHSR and are not included.
