Homes CMS first approved to provide Medicare and Medicaid in the last 15 months, newest first. Federal rules cap the gap between standard surveys at 15 months, so a home inside that window may not have had its first recertification survey; a home certified after an initial survey already shows it. Nothing here is a rating, and a home leaves the list by itself when its certification is 15 months old.
