These findings are about the building and its equipment as the health surveyors see them — a different survey from the Life Safety Code visit, and usually the maintenance department’s to fix.

57% of the facilities cited on F-0689 have been cited on it again at a later survey. A finding that recurs at the next survey is usually a gap in the routine that produces the record, not a new fault — the same inspection is missed, or completed and not documented in a form the surveyor accepts.
